HELP!! Need copy of UPC code for Radeon 64 DDR

I need a copy for a rebate I messed up on. Bought the card from BB and sent the original UPC to the ATI rebate that was exclusively for online purchases. So they denied my rebate and can't return my original UPC. Best Buy rebate center said I could send them a copy and they would honor it. I've tried BB, OD, Fry's...none left, must be discountinued.
