Lift the sticker with the panflo name on it(center of fan hub).
Add a couple drops of oil and top off with a dab of grease(toothpick end). While you do this take care to put to lubricants in the hole.
Replace sticker with black electrical tape. Plug in and enjoy silence once more. If not your fan has thrown a bearing and your should start shopping for a replacement.
