HELP!! Identifying bad memory?

I have a 64mb stick of PC100 SDRAM that went bad, and it does not identify the manufacturer. These are the only 2 labels on the chip:

Label 1) "905.11 - Warranty void if removed"
Label 2) "123900048089" (barcode) "64MB PC-100"

Is there any way to identify the manufacturer (can't remember where I bought it)?
 
Check the inscriptions on the chips themselves....

There should be some symbol of some sort.

MT = micron

There are others. If you can determine that symbol and post it, I'm sure someone can tell the manufacturer..
