Help building a DC -> DC converter for a laptop

I want to go from 12v cigarette lighter to 19v DC laptop input.

I'm not really 1337 at this, but I think the basic idea would be:

Lighter -> transformer -> laptop

anything else I would need to put in there? Would I need anything to clean the signal? How should I deal with the car not putting out exactly 12v (as it's normally more like 14v).
 
Easiest way would be to get the Auto/Air adapter for your laptop. A cheaper and more universal way would be to get an inverter which provides an AC outlet to which you plug in your normal AC adapter for the laptop. Make sure the inverter puts out enough watts (45W, 60W, 75W).
