Seems to me that if you are writing up a document and want to include the maiden name of your spouse would use the word "ne" as a prefix. For example, if you are John Jones and married Jane Smith, if you needed to write a document that showed her maiden name it would be something like "Jane Jones (ne Smith)"
Is that correct?
