Having Problems with Compact Flash Memory Card

I have a 128 MB compact flash card from Sandisk that I use on my Canon camera. Today, I took a picture and the whole LCD screen went white. So I tried to turn it off, and when the camera did not respond, I took off the battery. Then I tried to put it back on and turn on the camera only to find out that the compact flash card was not recognized. I tried to format it but that did not work either. The camera gave me some sort of error. So I put the card in the card reader and tried to format it there, but windows would no recognize it. Any ideas as to how I can reformat the card? I am not worried about recovering anything on the card itself. Thanks for the help in advance.
 
Any constructive advice? (Please not one that involves making an art work of some sort with it, or a key chains and whatnot.)
