This is the direct, predictable result of "Don't ask, don't tell". :|

She gave her life to protect Harry, which put a spell over him so that Voldemort couldn't touch him.

So, by Voldemort having Harry's blood in him, the spell was also put over him, making them seem like the same person...ish.

It's like Harry being in a force field, and Voldemort being on the outside. Once Voldemort got in, there was nothing left to protect Harry.
