Hard Drive Won't Initialize

I have a Western Digital Scorpio Blue 500GB WD5000BPVT 2.5" hard drive that i can't get to work. When ever i connect the drive to my desktop to format it a screen pops up and asks to initialize the disk but when i click ok a screen pops up and says function error. When i did get it to come it it says Manufacture: Unknown
Disk Size: 0/0GB


I have connecte directly to my motherboard and by using a HDD enclosure to no avail. Could someone please help.
 
Does it spin up?

Can you see its correct model number and full capacity in BIOS?

Can WD's DataLifeGuard detect it? Do the SMART data look OK?

What does HD Sentinel say about the drive?

1.) Yes

2.) No only shows as Unknown (0.0GB)

3.) Somewhat and it doesn't say

4.) haven't tried that
