I shouldn't be surprised that "Running Scared" is #5. The website for the movie had a flash game on it where you could perform oral sex on the main character's wife.
Come to think of it, it's probably a pretty big box office flop too. It really dropped off the radar.
Edit: I'm surprised "The Aristrocrats" is so low. There were things worse than the f-bomb said in that movie though, so for overall vulgarity, it'd be pretty high.
