Just tell them that you are everything you dreamed you would be and more, and you deserve a super-raise.
 
If you think you can get away with it, refuse to do it.

I refused my last one at my last job, I got my raise anyway & they never really cared.

I enjoyed them when I was working in the larger companies. It really gave me an opportunity to shine, because I was finally able to express ideas/etc. to those who had the power to act, not just listen [and ignore]. If they're just sending you a form to fill out and send in, then I'd say forget it. If they really cared they'd let you have an actual conversation.
 
They already know in their head how you stand and what raise you get. Just don't make waves. Write some typical BS and get on with work!
 
an old guy that used to work for me got one of those self eval forms. first question: what is your goal for the coming year? his reply: "to work here for another 12 months without getting fired". he left the rest of the form blank. then he quit 8 months later 😉
