Graphics are garbled. Why?

I've noticed on some software the graphics are garbled. For example, on the buttons, a red X appears and there is not test where ther should be text. The software is okay on other computers.

Also, the banner on ATT Worldnet has the same problem.

I have a Voodoo 3 3000 VC.

Anyone know how to fix this?
 
are you overclocking?

Is the card being cooled in any way?

do you have the latest drivers?

Have you tried a different video card / monitor?
