I just had to pay the co-pay for my insurance, so $10 I think.

I had it done at my normal family doctor. They just take a couple vials of blood and send it off to a lab.

I was tested at 8AM, and hadn't had breakfast, so probably around 12 hours. They didn't give me any special instructions when I made the appointment, so I'm not sure what they expect you to do in that regard.
