TheBlindMan posted the simple solution here. Basically you HAVE to choose a silly icon (below) before replying to the thread. I just choose the standard white page icon and it WORKS!
 
I'll need quite a bit more information than thats given here. By default note.gif is selected on ANY post,reply,edit form.
