Windows XP, I'm aware that Vista has parental controls built into, although I don't know the details of how it works at the moment.
Basically the same company that offer Iprism filtering devices to corportations. The only drawback is if someone has admin access they can change the DNS to point whereever. I use this along with Vista controls and it is pretty effective. Even works with dynamic IPs. The best thing is you never have to worry about updating since they take care of it.
EDIT: this is not a software package. You set your routers to resolve ips from the opendns servers. Nothing to install. You create an account online and manage settings from there.
