Full duplex Fast Ethernet: Explain?

I know that you can only get full-duplex with a switch/multi-NIC router, but what exactly is going on?

Some say you effectively get 200Mbps, 100Mbps upstream and 100Mbps downstream. Why can't two cards and a crossover cable achieve this? Do they not have dedicated transmit and receive wires which is why you must cross a crossover cable? Does it work and improve performance between any two computers on a switch or does it require three+? ie, one PC (PC1) uploads to the server PC (PC2) at 100Mbps while another PC (PC3) downloads from the server PC (PC2) at 100Mbps
