Free PCB design software?

Since it's used to design hardware, it's not a software question, per se...

But, what is the best free PCB design software that you guys have used. I'm using Eagle right now, but I need more than the 4" x 3" limit the free version imposes.

Do you have to use PCB express to fabricate your boards if you design them w/ this software?
