The questionnaire is hosted on Zoomerang, and should only take less than 5 minutes. It?s for a school research project at UC Berkeley, and the project sponsor, Norcal DJMPA, has personally guaranteed all survey participants will have their free lessons honored.

The deal gets even better ? the one-time lesson allows you to bring a friend for free also. If you decide to enroll in the classes afterwards, a 10% discount can be applied to all classes for you and a friend.
