FPS Caps and Refresh Rates

I was talking to people while playing CS:S today about our FPS (naturally). Many people expressed that they have FPS of above 150. While I believe some of them are most likely lying, I don't see how that many of them could be. With that said, I can only get a max FPS of 85 (with all the fps_max tweak crap things) because my monitor is set to 85Hz refresh rate. I don't see how other people's cards are capable of surpassing their refresh rates while mine seems to determine my FPS caps.

(Not just in CS:S, mind you. World of Warcraft does the same thing as well as Condition Zero.)
 
Cap is only present if Vsync is on. If you turn Vsync off, there is no cap unless the game engine has one.
