With pulleys and a v-belt?

The friend that sent it confirmed that if you jerk the steering wheel the belt slips.
 
Yep, I too have seen them locally listed around here, and we have state inspections.

Edit: Yes it was very similar set up to that.

I could see it being workable with a toothed belt or a chain but a belt that's able to slip just seems like Darwin waiting to happen.
 
My current mailman drives a standard car. He sits in the passenger seat, with one leg over the console for gas and brakes, and steers with his left hand. I've seen paper delivery drivers do this too.

Doesn't seem like that would be very safe either.

Yep, nearly exactly like that. If the belt slips, it needs replaced. Hardly an inspection problem IMHO as if the car is no longer in service in an official capacity, the extras should have been removed before it was sold.
 
here the mailmen just use scooters with a cart since they often have to go into pedonal passageways because the mailboxes are on the entrance, not the street like in the US.
Even those close to the street usually require you to get out of the car since single mailboxes are a minority.

also the police doesn't tolerate cars like that, every modification requires you to go and make a new car test to check the safety.
