I should warn you - any mount you use with a 50" plasma will likely sag a little, so you may be better off with more of a flush mount than an extendable one. I mounted my 50" plasma on this mount, but I leave it pushed against the wall. It sags when extended, but not horribly so and I don't leave it extended anyway. I wanted a mount that would let me conveniently pull the tv out to get to the back and change cables around, without taking the tv off the wall.
The answer to your question about cables is that I ran mine through the wall, using a fiberglass rod to push through the insulation. I got the rods (three 5' lengths that can screw together if necessary) from Home Depot, I think. They were pricey ($45) but I use them more than once.
Once concern about wires through the walls is the power cable. It is a shame to run wires through the walls but then have an exposed power cable. But it is usually against code, I believe, and something of a fire risk to run a power cable through the wall. Just something to consider.
