Floppy Disk Fail (40) -- What gives?

I'm using an Epox EK7A+ with a two drive RAID0 configuration. :)
I have booted with two different floppy drives and with no drive, and I get the message
Floppy Disk Fail (40). I can't load w2k b/c it requires that I load drivers from Drive A. Drive A allows me to read the directory sometimes, but I can't boot from it or load drivers for w2k.
Oddly, Bios shows that Drive A is a 3.5" 1.44MB drive even when the drive is not plugged in.

What's going on? How do I fix this?

check to make sure you don't have the Floppy cable backwards. If that fails, try a different cable all together.
