According to Covad, Flashcom has filed for bankruptcy protection. If you try and use the Covad safety net service, you get the following message:

At this time, Covad's policy towards ISPs in bankruptcy is to not switch their customers to other ISPs without the approval of the bankruptcy court or the written consent of the ISP itself.
