filtering grainy images?

Has anyone used any kind of software that works well for filtering graininess from pictures?
Anything I've come across so far either doesn't work that well or makes the images noticeably blurry.

Something free would be pretty cool but, if there's something with a reasonable cost that has a demo I can try out, I'd be willing to pay the price. 🙂
