The half-trained monkeys who work at tire shops usually just grab the impact wrench and gun it when they put on tires, instead of starting them by hand and finishing them with a torque wrench like you're supposed to. I can easily see how they could make that mistake.
But I don't see how you have to be "far more mechanically inclined" than Average Joe Racer to recognize incorrect threadings when you feel them.

wait, so was the lug 1) too large for the stud? or 2) vice versa?
in case 1) even with an impact wrench the lug will come right off.
so is case 2) applied to this incident?
