Put it on a copier (Kinkos) and cut out the template. 🙂
 
A dremel if I am away, or a hole punch. Any machine shop will have one.

In the past I have also used hole saws (drill) and then cleaned it up with the dremel.
 
Another method that isn't quite as easy but is cheaper and you can do at home is tape the fan to a piece of paper, flip it over, and gently 'scribble' an image of the parts you need. Also eliminates the possibility of any resizing the copier might do (you never know).
Using the screwholes as guides, draw an "X" to locate the center for using a hole-saw (quickest, easiest, cleanest method for beginners, trust me).
