Fake/embarrassing survey EDIT: FOUND

Does anyone remember a form based "trick" survey that actually sent the results back to a specified email address? I remember one of my friends sending it several years ago, like maybe 2000. It basically had a bunch of embarrassing questions and was supposedly anonymous, but when you submitted it it got sent to the person that originally gave you the link.

That actually happened to me this year. I don't remember what it said but it sent it to my friend, haha.

Why do you ask? It would be simple to make if you wanted one made.

Yeah, it's just downright mean. For some reason I thought of it when I was driving to work today. I heard something on the radio about that site, or something similar, and it reminded me of the one I had seen. If it's on a morning radio show, I guess that means it's making the rounds again.
