Ever have to fill out a form to buy liquor?

There is some chain liquor store around here in NJ called bottle king that has these forms they sometimes ask you to fill out, name, birthdate, address when youre buying booze. apparently they only make you do it when they ask to see your id, and you have to do it every time they ask. They say they ask anyone who looks under 30 for id. Anyone else ever dealt with this?

The first few times I went in, no problem, no asking for ID(i'm 22 btw). But the last couple times they've asked and I've had to fill out this form, and its quite annoying. I know its not that big of a deal, but its just so stupid it gets me pissed off. I believe they also write down your license number on it? I guess this is to stop fake ids, but I don't see what good it does. If its fake, you still sold the person the booze. I've never seen this anywhere else.

Would you guys just deal with it? The place is a decent amount cheaper than anything else in the area and its a pretty nice store so I haven't made up my mind yet lol. I'll probably get flamed but whatever, have fun

The only forms I've ever had to fill out at liquor stores were the keg registrations. Never heard of forms for any random stop.

I'm in MN.

I live in Oregon so all the liquor stores are state run, even there I've never had to fill out any forms.

Yep. Apparently they don't quite trust your ID as actually being yours...or accurate.

Once upon a time...when I was a yout...I had to fill out a form similar to that. The clerk didn't quite believe my ID was actually mine. (which it wasn't :D )
This takes the liability off of them in many states...some states will still nail them for selling to a minor, even with the sworn affidavit.

Yeah Bottle King does it all the time. Sometimes they dont even ask for my ID and other times they make me fill out the form. It depends on how lazy the cashier is that day I guess. The form is just saying that they have you on camera buying it and if you serve it to underage people you can get in trouble blah blah blah. They have the best prices in the area though so its worth it I think

I've had to do it in PA, probably since I looked underage (22 at the time) and my license was out of state so it wouldn't scan.

This gives them all the information that they are taking from you on the form..or are you just complaining about the time, and not giving them the information?

I wish someone would check my ID when I bought liquor ... but that stopped a long time ago.

I haven't been carded since 1992 ... and I think that was just a pity check to make me feel better. Oh well.
