I have reason to beleive the issue is NOT hardware, but rather something wrong with windows / or an update.

WDC data lifegard will not run. Error 0201- Missing Test Track
Common error reported online. WD messed up. However the windows version of the diagnostic works fine, and passes on long test.

Dell diags passes long hard drive test.

How can I troubleshoot if it is a windows problem.

Hard drive is a caviar blue 250GB WD2500AAKX

Is that the only error you see in the event logs?
If you have another HD, I would copy all the data from that one, then format it again, and see if it still has those warnings.

If you want to rule out windows, then, I guess you could do a clean install of the OS, and that should fix anything wrong with your windows install.
Of course, before doing that, it is wise to test your RAM with memtest86+ and make sure you have no errors. Once that is ran overnight, with no errors, then you can do a clean install of the OS.

I'd run a chkdsk on your hdd to test, to start with. Then, I'd run a sfc /scannow to verify windows is proper.

Yep, chkdsk fixed it. But I want to know what caused it to begin with. HD passes diags.

Improper shutdown
Bad HDD
Power surge
Bad ram
Bad cpu
bad cables
bit rot
etc
etc

NTFS is a tried and tested file system. It isn't really known for corrupting its own file structures unless something else goes horribly wrong and writes cannot get flushed to the disk properly.

So I know all of this. Been using XP since 2001, so familiar with filesystem.

My question is HOW to diagnose WHAT is causing it.

Power failure / improper shutdown is ruled out. On a battery backup since new.

HDD tests good on long DST. WD has a bug in their offline program in which it cannot find a test track on the HDD. But the WD windows diag passes.

Dell diags pass.
