My friend also had same trouble for the Printer purchased on Nov-23.
He called Staples CustomerService.
With some difficulties, they faxed him the rebate form.
caution - this printer is an ink guzzler. don't leave it plugged in when you don't need it. every time you boot your computer, it cleans its its printer heads, hence depleting the ink supply. if you never reboot your computer, it's of course a moot point. but of course if the printer is so cheap, it may be worth it to risk voiding the warranty by using cheap knock-off generic ink.

Is that what it is doing? I wondered what was happening to my ink supply. . . . thanks for the tip!
I believe the warranty is only 90 days . . . you bet I'll get the generic replacement ink.
Call the Staples rebate center and tell them your situation. I was in the same boat for the Brother Label Maker rebate a couple months ago. I ordered within the specified time frame, but by the time my order arrived (so I could enter the UPC online), the form was gone. The CSR at the rebate place simply checked with his manager and then approved my rebate over the phone (no submittal at all!). As always, YMMV, but its definitely worth a shot.
