Email from friend not sent by him

So I got an email from a friends email address, but he didn't actually send it. He said he got an email from him self as well.

So is it likely his email got hacked or could something else have happened?

He still has access to his account.
 
email security is pretty weak, i'm pretty sure most free webmail has been compromised in some way or another, somehow they got his address book or whatever. email spoofing is pretty common, like writing from santa claus on an envelope and tossing it in the mail.
