My guess is that the phantom power (48V) was turned on at the mixing console...this feeds electicity down the line to give power to condensor microphones which he could have been using.
But this is delivered via a very high resistance - as such, any injury from this would be very unlikely.
The only plausible scenario was that:
1) The amp was faulty (degraded insulation between mains input and electronics)
AND
2) The electrical installation in the church was inadequate (no residual current breaker) or faulty.
