Costs are pretty insane in the US, but part of how costs are decided (at least the list prices, and not the true prices people pay) is how much systems can save with X treatment vs existing standards. It isn't completely arbitrary. So how they decide to set a cost is often based on how much can be saved via reduced hospitalization, patients living the ability to work, people needing fewer ancillary meds...

At the same time, many people may not like healthcare as offered in some other countries, because their cost controls often come from telling companies "no". But then you never see the insurance company deny a claim because the doctor knows the med is not reimbursable, so they may not bother mentioning it.
