DVD+RW has failed and its color has changed!

I burned Vista Beta 2 on a DVD+RW DVD and used it a few times successfully to install Vista.

After about a month, I attempted to clear the DVD and burn Vista RC1 on it. It failed. When I got a fresh DVD-RW to try, I noticed that the color of the failed DVD-RW has changed.

The one on the left is the one that has failed and does not work. The one on the right is the one I just burned and works fine.

Why did the DVD fail?
Why has the color changed?
 
It seems to me that the burning laser in your DVD burner has gone berserk. Color should not change. Looks like it got seriously burnt.

Looks like a drive replacement is in order. What kind are we talking about here?
 
The drive is NEC DVD_RW ND-3500AG.

Both DVDs in the picture are burnt. So, if something was wrong with the burner, wouldn't both of them look like that?
 
Looks like it could be bad media, and or, how it was stored. If the laser was screwing up, 1 would expect all your DVD's to be burnt.
 
The only difference between the two is that I attempted to erase one and burn with a different image (using Nero). That's the one that has color on it and has failed.

I would definitely say something is wrong if came straight out of the burner like the left one. You said you used it and it became bad. Now just remember where you had it.

After burning and using it for installing Vista, I had it in an individual case on a CD/DVD rack. It was not exposed to direct sunlight. The problem is that I do not recall exactly when the color changed. But, I will keep an eye on the other one. I wonder if something went wrong during the erase procedure!
 
Ask Memorex - it would be extremely unusual if someone here had a similar experience. At least you might get a replacement disk out of it...

I wanted to make sure that this was not something obvious before I contacted them.
