Back when people used cassettes, you actually had to buy head cleaning tapes, and runt hem in the machine every now and then.
Since the laser never touches the lens, I am skeptical of claims from manufacturers that you need to clean the lens.

However, our DVD player seems A LOT pickier about what discs it will play, and it seems to hang, stutter, and pause a lot on discs that look pretty good to my eye. The only explanation I can figure out is that either the lens is slightly misaligned, so the tracking tolerance is not quite at Red Book ( Red Book link at Wiki) levels, or the lens is dirty.

They have a little brush embedded in the disk so when it spins, it brushes off the dust on the lens.

I could see it getting dusty if it's not sealed up all that well. Otherwise I'm not sure why you'd need to clean it.

These used to work on CD players back in the day but only as a temporary fix.

The lens does collect dust over time and it definitely can effect how well the player reads discs. A $10-20 cleaning disc usually takes care of it.
