Driver's Test In California

Those who live in California, you guys know that u need to do the 50 hours, 6 of which at night when you got your permit. Well am i supposed to make my own paper for this? Like take a piece of paper and write down all those times i was diriving? Or do they give u a special form at the DMV, and you need to copy down all the times you went driving? Or do they just make you sign up a paper saying that you did it...?
