Doesn't make sense

My FIL is runing XP with 128MB RAM. He was having virtual memory errors. He upgraded to 256mb RAM. He's still getting the errors. He has 49% free space on hard drive. He is only surfing the internet and using Outlook when this happens. V.M is set to allow windows to adjust.

He has run spybot and AVG and housecall virus scans. System is clean.

What could be causing this?
