Google hosts mail for my employer's @numail.org domains. One of our users just told me that his "Settings" and "Sign out" links disappeared from the top of the webmail page.
Since our company switched to Google mail hosting, I've noticed that each user randomly has one of 2 different interfaces. One looks just like my Gmail account, and the other looks a bit simpler. The simpler version has a plain "Settings" link where the other interface shows a picture of a cog. I switched to a test mail account that has the simpler interface and noticed that the links were there, but they were invisible (white text on white background). Some alternate themes make the links visible again.
Google's help system isn't much use. There was no appropriate category to select and, after filling out way too much information, the submission failed with: "Oops! Try again in 30 seconds. We don't know what went wrong."
As ALWAYS with this kind of thing, the whole form was blank when I clicked back.
