Does anyone know what font this is?

I'm trying to recreate an old beat up form and it needs to be replicated almost exactly. For the life of me I cannot figure out what font was used? I'm guessing it may have been done on a typewriter so maybe there is nothing that will be able to replicate it?

I would just scan it/PDF it but the form is way to beat up.

Ok, so how did you do that so fast? Are you guys just real big fans of fonts or is there someway other than looking at huge list of fonts and trying to find something that matches?
