Does anyone keep their workout log in a binder?

I've been using a notebook for the last two months or so and my information is starting ot get really unorganized. I was thinking of keeping my daily weights log in a binder so I could track my weight increases and make sure that I'm making constant gains.

What sections do most of you use in your log binder?

I was thinking of putting

Daily Logs (where I'd log my cardio, weights, etc)
Exercises (where I'd have the written description + pictures of my workout, and the notes I keep regarding technique.)
FAQ (I have a trainer, so I usually ask her questions, or she makes some important comment sabout things I need to know. At the moment I'm just writing it anywhere in my book, I'd use this area to keep access to some of the questions that I asked her or notes she told me to take.)
Research (where I'd keep anything I got off the net)
Pictures (an area to keep my weekly pictures to track my progress.)

Anyone else who uses this method have any other tabs they think are important?
