Does anyone have instructions for redeeming Mountain Dew caps?

I have this $7 gas cash cap I'd like to send in, but the page on their website with instructions isn't there anymore. Does anyone have the page saved or something?

I haven't seen any bottles like that recently, they may be out of circulation. If I was going to go to all that trouble I might as well call them and ask for directions.
