I've never cited anything really ancient. The oldest citation I've made would be around 18th century, and yes I italicised the titles.

there's just about no reason to underline something for citation purposes while using a computer.

(could be wrong, but I was told as an undergrad that the only reason underlines were ever used at all was because back in the day, not every typewriter could do italics)
