Yep, I'd also say Fallout 3 was the game I had the most difficult time with going totally crazy in. There are just huge consequences to your actions, and some of the ones you'll make playing "pure good" and "pure evil" are going to be detrimental to what you want as a player. Even as an evil character, nuking Megaton took some real willpower, because it's simply a very handy outpost, and the reward for doing it doesn't really seem all that great.
