Yeah, I can get it really bad.

Good flossing, thourough brushing, and my secret weapon: rinse with 1 part water, 1 part hydrogen peroxide.

Edit: I sometimes use Listerine after that, just because it actually tastes less nasty. Note that a meal and a cup of coffee can completely reverse the goodness.
 
Floss. The bacteria that causes bad breath resides in the plaque at the base of your teeth and in the food left in between them.

Flossing regularly eliminates the bacteria as well as the stench.
 
No, but I know several people who do. It's amazing how someone's breath can nearly knock you down when you're just sitting next to them and they're mouth isn't even pointed in your direction.

an ex-boyfriend of mine has awful breath, but when we broke up, we became close friends and i never wanted to tell him since i didn't know how to do so without embarassing him completely.

well, the time came where we parted (un)amicably and i finally told him that his breath stinks and that he needs to get new toothpaste. i am seriously wondering how he's handling it 🙂

It can also reside on the tongue and under the tongue. Both of these areas should be brushed as thouroughly as possible. I read somewhere that some people can have especially rough "terrain" in these areas, and that these people often have bad halitosis.

This came from a website from a dentist who was trying to sell a special brand of mouth rinse that "foamed up" as it killed the anaerobic bacteria. This is what led me to try HO, which worked like a charm. I'm betting that this guy is just selling HO relabelled as a miracle cure.
 
Well my breath does smell like a rat's ass but I have come ot realize this is the case for almost all adults. As people get older their breath just turns into a butthole, even if their dental hygiene does not diminish. It can be difficult to cure, and is based mostly on stink from the tongue (for those interested). Many people can brush their teeth and 5 minutes later smell like a rotting fetus. It's best controlled with tongue scraping, cleaning the teeth, possible use of oxygenating (I think that's it) mouthwashes, and lots of gum to cover it up.

It's bad right now. I had a philly steak sammich with onions, jalepeno's, green peppers, and provalone for lunch.
