Do I need extended ATX?

Nah, it more depends on the motherboard you get. MSI's dual-opty board is standard ATX, as is the Tyan Tiger K8W. If, however, you want one of the more expensive (and larger) Opteron boards, such as the Tyan Thunder K8W, you will definitely need a case that is large enough for Extended ATX boards.

For a few months, I had a dual Opteron machine with the MSI board, and it fit nicely in my Compucase LX-6A19, which is very similar to the Antec Sonata. I had no problems fitting the board inside the case. If you do decide on an ATX sized Opteron board, just make sure that the mid-tower case you pick has good airflow.
