Distorted CD's, skipping, noisy mess!

I've tried burning with Nero, MMJ, CDEX, CD Creator; I've also tried creating MP3's on the HDD. The sound quality is the pits, along with broken threads, noise etc.

The sound quality is A+++ when played directly from the CD ROM.

Maxtor 30 Gig HDD w/2 Partitions OS/Other
Sony 48X CD ROM
HP CDR/W
ESS Maestro 2 PCI Soundcard
