Question Display port to HDMI cable or adapter for 4K @60hz TV?

I'm getting mixed results searching online. I have a Radeon RX 560 with HDMI, Display Port, and DVI. I have a 1080p Freesync monitor (HDMI and VGA) that supports 75hz and a 4K TV I want to use as a second display. For what I understand the monitor has to use the HDMI port because of Freesync and 75hz. That leaves me with either the Display Port or DVI to use for the 4K TV. I need video and audio going to the TV so I can't use DVI. Is there a way to go from the Display Port from the video card to the HDMI on the TV at 4K 60hz?
