DirectX / D3D issue

New build, experiencing what I think is the same issue, but in two places:

Skype: webcam freezes / goes blank after 15 minutes. Does this until restart my machine.
D3D failed to create device

Diablo 3: error arises when I Tab out / background the game. Does this until I force-quit and restart the game.
D3D failed to create device.

-I have updated motherboard drivers
-I've tried different graphic card drivers
-I updated to the most recent DirectX (11) three times.
-I've run Dxdiag. Sometimes it works flawlessly, sometimes it says,"DxDiag has detected that there might be a problem accessing Direct3D the last time this was used." Then it says "DxDiag has stopped working."

So what should I do next? I can't go on like this!

-Windows 7
-Asrock z77 mobo
-AMD 7850 gfx
