Direct CD Problem: How am I ruining mt CD-RW disks?

I put the cd-rw in the drive and direct cd tells me to format it and I do and then at the end it tells me the disk is bad. 2 in a row. what is up here?

is this a quick format or a full? Try using Nero as a last resort.
