I go by my name on my current birth certificate (issued at about 5.5 months old.) I do not go by the name on my original birth certificate or by the name I was called prior to my current birth certificate ("baby girl" and "Amy" respectively.)

Edit: I stand corrected, I looked at my original certificate just now and saw that the first name and middle name lines were left blank.
