He injects a certain liquid into his arms to make them bigger. Not Synthol but a combination of 2 other ones. I can understand doing it to a certain point but not that outrageous and disgusting looking
 
from what i've heard he used to do synthol injections... like crazy. now he's finally took the next step and got implants. nobody with arms that big would have forearms that small and such round, misshapen, smooth skin where striations should appear.
